How to Write a Nursing Care Plan
Step 1 — Assessment: Gather subjective and objective data.
Step 2 — Nursing Diagnosis: Problem related to Etiology as evidenced by Signs/Symptoms.
Step 3 — Planning: Set SMART goals.
Step 4 — Interventions: Independent, dependent, and collaborative actions with rationales.
Step 5 — Evaluation: Were goals met?
Common Nursing Diagnoses
- Impaired Gas Exchange
- Acute Pain related to surgical incision
- Risk for Infection related to invasive procedures
- Deficient Knowledge related to new diagnosis
