Shadow Health Tina Jones Health History — What questions should I ask?

I am doing the Shadow Health Tina Jones Health History assessment and I keep missing points. Can someone tell me all the questions I need to ask to get a high score? I need help with the HPI, ROS, PMH, medications, social history and family history sections.
